A companion to the UK failures index, this page collects cases in which the failing system’s relationship with the natural environment is central: air and water pollution, radioactive and chemical contamination, and biosecurity. The boundary is deliberate. It cleanly includes hazards that are environmental in kind, while pushing floods-hitting-infrastructure toward a future infrastructure-interdependency theme and leaving industrial disasters with environmental consequences (Aberfan, Buncefield) as border cases to be decided individually.
Environmental failures are worth gathering separately because they sharpen patterns the rest of the archive only hints at. Their harm is often invisible, latent and diffuse — spread across a population or a catchment, and unfolding over years rather than in a single event — which makes them unusually hard for an organisation to perceive, to attribute, and to admit. And they surface one pattern not yet in this site’s concept set at all: cases where the intervention becomes part of the failure.

Cross-cutting patterns
Reassurance in place of disclosure
Camelford, Windscale and the Great Smog all show authorities choosing to avoid public alarm — or to protect a programme or an industry — over telling people plainly about a hazard. It is the BSE pattern in an environmental register: certainty manufactured from uncertainty, with trust as the eventual casualty. Environmental hazards are especially prone to this, because the harm is often invisible at the moment the reassurance is given.
The intervention as part of the failure
Torrey Canyon is the clearest instance: the toxic dispersants and the bombing of the wreck did much of the ecological damage the response was meant to prevent. The 2001 foot-and-mouth cull has the same shape — a control measure whose scale and manner became a second crisis alongside the disease. This iatrogenic pattern, where the remedy amplifies the harm, is now developed as its own concept — see iatrogenic failure.
Chronic, distributed, invisible harm
Air pollution, and arguably the Great Smog and the sewage crisis, inflict harm that is too slow and too diffuse to register as a discrete “failure event.” An organisation that perceives the world through incidents is structurally blind to damage that has no single moment — a perception failure in the sense developed under organisations as cognitive systems, and a reservoir of latent conditions that never quite crosses the threshold of official notice.
Financialisation and regulatory capture of natural monopolies
Thames Water is the live case: a privatised utility optimising for financial extraction while its environmental obligation — not discharging sewage — went unmet, overseen by a regulator that could not or did not compel it. The kinship is with Carillion on financialisation and with BSE on capture, applied to a natural monopoly whose failures fall on a shared river rather than a balance sheet.
A natural trigger meeting a brittle system
The 2007 floods sit at the boundary of this section: the environment as the trigger that exposes infrastructure interdependency, with the near-loss of a single electricity substation threatening power to half a million people. It bridges toward tight coupling and toward the infrastructure-interdependency theme that this and the 2019 blackout would anchor.
